肺腺癌を合併した微小髄膜細胞様結節の1例

A case of multiple minute pulmonary meningothelial-like nodules combined with adenocarcinoma

微小髄膜腫細胞様結節（multiple minute pulmonary meningothelial-like nodule：MPMN）は，肺の組織に発生する微小な結節で，組織像，超微形態，免疫組織化学的には髄膜腫に類似している。従来は剖検肺や手術肺で偶発みつかる微小病変であったが，画像診断の進歩に伴い，胸部CT所見を契機に発見されるようになった。本症例は，胸部CTで両肺の多発結節を経過観察中に，1つの結節が増大したため，胸腔鏡下右上葉部分切除が施行された。病理組織学的に増大した結節は乳頭状腺癌，結節の1つは上皮内腺癌，残りの結節はMPMNと診断された。肺腺癌を合併した微小髄膜細胞様結節の1例を経験したので，文献的考察を加え報告する。

A 66-year-old woman was referred because of multiple nodules in both lungs on computed tomography. One of many nodules grew, and video-assisted thoracoscopic surgery was performed. We diagnosed the patient with adenocarcinoma combined with multiple minute pulmonary meningothelial-like nodules（MPMN）. MPMN are ground-glass nodules, ranging in size from 1 to 4 mm in diameter. It is difficult to distinguish MPMN from malignant tumors, and MPMN sometimes accompany malignant tumors.
