共同意思決定（Shared Decision Making）に基づく小児アレルギー看護実践項目とコアとなる要素の明確化に向けた質的調査—個別面接とフォーカスグループインタビューを用いて

Qualitative Survey toward the Development a Draft of Items and Core Elements of Pediatric Allergy Nursing based on Shared Decision-Making in Clinical Practice: Using Individual Interview and Focus Group Interview

要旨

　目的：本研究の目的は、共同意思決定（Shared Decision Making：SDM）に基づく小児アレルギー看護実践ガイドラインの開発にむけて、看護実践項目の草案を作成し、コアとなる要素を明らかにすることである。

　方法：小児アレルギー診療に携わる多職種の医療者、アレルギー疾患をもつ患児と家族への面接調査を実施し、看護実践項目とコア要素について質的に分析した。次に、小児アレルギーエデュケーターへのフォーカスグループインタビューにて、分析結果の妥当性について検証した。

　結果：アレルギー疾患に共通する64項目と、疾患特有の29項目（計93項目）が抽出された。コアとなる要素は、【目標共有のためのコミュニケーション】【疾患と治療の理解】【セルフケア支援】【日常生活の把握】【意思決定支援】【役割認識と連携】の6つであった。フォーカスグループインタビューにおいて、6つのコア要素に対するコンセンサスが得られた。

　考察：看護実践項目はSDMの概念、SDM達成のための具体的な実践と合致しており、加えて、小児アレルギーエデュケーターに求められる患者教育のための到達目標を包含していることが確認された。

　Objective. The aim of this study was to clarify a draft of pediatric allergic nursing practice items and its core elements for the development of guidelines based on shared decision-making (SDM).

　Method. We conducted an interview survey with a multidisciplinary team including pediatric patients and their families. Next, a focus group interview was conducted for pediatric allergy educator (PAE) to obtain the opinions on a draft of nursing practice items and core elements.

　Results. Sixty-four items common to the allergy disease and 29 items specific to the disease (93 items in total) were extracted. The core elements were “Communication for sharing goals,” “Understanding of the disease and its treatment,” “Supporting patient self-care,” “Understanding patients' daily life,” “Supporting decision-making,” and “Knowing own role in collaboration with multidisciplinary team.” A draft of items and six core elements were endorsed by the focus group interview.

　Discussion. The nursing practice items were consistent with the concept of SDM, and concrete practice for achieving SDM. In addition, it was confirmed that it encompasses the goal for patient education required for PAE.
