精神科作業療法における治療構造—精神分析学的側面からの検討

Therapeutic structure in psychiatric occupational therapy: Reviewing the psychoanalytic perspective

要旨：精神科作業療法(以下，OT)の中で精神分析が取り上げられることが少なくなっている．しかし，精神分析が今までに築いてきた知見を参照し，OTの治療構造に新たな視点を持ち込むことが必要である．精神分析の対象は神経症や人格障害などが多く，無意識の意識化と洞察を目指すものである．一方，OTでは統合失調症や認知症などが主たる対象で，症状，対人関係，日常生活，社会生活などの障害への援助を目指している．このことから，精神分析の知見がそのままOTに援用できるとは限らない．しかし，対象者理解や治療内容を考える上で参考になる部分は多い．今後，精神分析の視点に立ったOTの実践例を積み重ねることが重要である．

　Some psychiatrists and clinical psychologists have recently suggested that the practice of the psychoanalysis has declined, and this is similar in psychiatric occupational therapy.

　However, it is important for therapeutic structure in psychiatric occupational therapy to apply new and accumulated findings of psychoanalysis.

　In this article, we try to 1) compare concepts and techniques of psychoanalysis with therapeutic structures in psychiatric occupational therapy, and 2) discuss the relationship between both therapies.
