Abstract : It is very important that we pay careful attention to preventing falls in hospitalized patients because falls can have immediate disastrous results such as fractures and head injuries. The rate of falling incidents in Tottori University Hospital among all incidents from April to November in 2011 was very high (0.22%), with one patient dying due to apoplexy caused by a fall. In December 2011, a multidisciplinary fall-prevention team was organized comprising members from a variety of departments such as doctors from rehabilitation, orthopedics, and neurology, along with nurses, pharmacists, physical and occupational therapists, and a risk manager. First, we investigated the methods of scoring hazard risks for falls of inpatients using of fall-assessment tool at admission and an individual fall-prevention assessment. Next, we made a round of visits to patients in individual hospital wards who were at high-risk for falling. We then supervised all members of our hospital for fall prevention. Thus, the fall incident rate in our hospital was reduced from 0.22% to 0.18% in a one-year period.
