非機能性膵内分泌腫瘍と鑑別困難であった膵内副脾の1例—腹腔鏡下脾温存膵尾部切除術の有用性

A case of intrapancreatic accessory spleen mimicking non-functional islet-cell tumor:usefulness of laparoscopic distal pancreatectomy with preservation of the spleen

　症例は75歳，女性．胆石症の精査中，腹部にCT,MRI検査で膵尾部に造影効果を有する境界明瞭な2cm大の腫瘍性病変が認められた．血液検査で膵内分泌ホルモン値の上昇はなく，非機能性膵内分泌腫瘍を疑い，腹腔鏡下胆嚢摘出術と同時に脾温存尾側膵切除術を施行した．手術時間は5時間10分，出血量は360 mlであった．第2病日から経口摂取を開始し，術後合併症はなく第10病日に退院した．病理組織学的検査にて腫瘤は膵内副脾と診断された．腹腔鏡下尾側膵切除術は低侵襲であり，膵体尾部の良性から低悪性度病変に対し非常に有用であると思われた症例であったが，膵尾部腫瘤の鑑別診断として，常に副脾の存在を考慮する必要があると考えられた．

A mass lesion in the pancreatic tail was identified during the preoperative diagnostic images of the pancreas in a 75-year-old female who was referred to our hospital for treatment of cholecystolithiasis. The mass, measur-ing two centimeters in diameter, was well-demarcated and was enhanced by dynamic computed tomography. No elevations in hormone levels were detected. Following the diagnosis of cholecystolithiasis and non-functional islet-cell tumor complicating, we simultaneously performed laparoscopic cholecystectomy and laparoscopic dis-tal pancreatectomy, preserving the spleen.
