腹腔鏡下に摘出した小網ならびに大網に発生したCastlemanリンパ腫の2例

Laparoscopic resection for two rare cases of Castleman's lymphoma of the lesser and greater omentum:report of two cases

　Castlemanリンパ腫は，リンパ濾胞の過形成およびヒアリン化と，血管内皮細胞の増殖を伴う著しい血管新生が特徴的なリンパ増殖性疾患である．その好発部位は縦隔，頸部とされ，小網または大網での発生は稀である．また，その豊富な血流量と周囲組織との強固な癒着のために術中の大量出血に注意が必要で，これまでの内視鏡下手術の報告例では開胸・開腹手術への移行がみられる．今回筆者らは，37歳と59歳の女性の小網および大網に発生したCastlemanリンパ腫の2例に対し，超音波凝固切開装置ならびにベッセルシーリングシステムを用いて腹腔鏡下に摘出術を施行しえたので報告する．

Castleman's lymphoma is characterized by hyperplastic lymph node enlargement with abnormal lymphoid follicles with hyalinization and capillary proliferation with endothelial hyperplasia. It is located most commonly in the visceral mediastinum with increased vascularization. In some previous reports on thoracoscopic or laparoscopic surgery for patients with Castleman's lymphoma, conversions to open thoracotomy or laparotomy have been described due to tight adhesion and bleeding.
