　腹腔鏡補助下幽門保存胃切除（laparoscopy-assisted pylorus-preserving gastrectomy：LPPG）の有用性を検討する．対象および方法：最近5年間に当科で施行された開腹幽門保存胃切除（con-ventional pylorus-preserving gastrectomy：OPPG）18例と，2003年から導入されたLPPG 10例との間で，周術期および術後6か月以降の各種パラメーターを比較検討した．有意差検定にはt検定およびχ²検定を用いた．結果：手術時間はLPPG群で有意に長かった（LPPG群289.1±39.6分vs．OPPG群209.2±37.9分，p＜0.001）．出血量ではLPPG群で少なく（LPPG群172.1±73.3 g vs．OPPG群354.7±140.2 g,p＜0.001），術後の鎮痛剤使用回数もLPPG群で有意に少なかった（0.50±0.97回vs．3.22±2.76回，p＜0.01）．リンパ節郭清個数，合併症発生率，6か月後の体重減少，食事摂取量，愁訴では両群に差を認めなかった．結論：LPPGは有用な術式であるが，今後の課題はいかに手術時間を短縮するかという点である．

Purpose: The purpose of this study is to clarify the potential benefits of laparoscopy-assisted pylorus-pre-serving gastrectomy (LPPG).

Methodology: Among the patients with early gastric cancer in the recent 5 years, 18 patients who underwent conventional pylorus-preserving gastrectomy (OPPG) and 10 who underwent LPPG were compared retrospec-tively, regarding perioperative and postoperative parameters.
