腹腔鏡下に膵尾部脾合併切除術を行ったインスリノーマの1例

A Case of Insulinoma Treated by Laparoscopic Resection of the Pancreatic Tail

　患者は80歳，女性．主訴は頻回の低血糖発作．その精査と治療目的のために紹介され入院した．発作時の血糖は37mg／dlで，血管造影，angio-CTにて膵尾部のインスリノーマと診断した．腫瘤は膵実質内に存在していたため，術中超音波検査を用いて位置を同定し，腹腔鏡下に膵尾部脾合併切除術を行った．手術時間は3時間17分，出血量は10mlであった．腫瘍は9×8×7mmで，術後の空腹時血糖は115mg／dlとなった．術後19日目に退院し，現在も低血糖発作はみられず，外来にて経過観察中である．インスリノーマは単発，良性であることが多く，低侵襲である腹腔鏡下手術のよい適応と思われたので報告した．

A 80-year-old woman was admitted to the hospital because of consciousness disturbance due to hypoglyce-mia. FBS was 37mg/dl when she had consciousness disturbance. An angiogram and angio-CT showed a tumor in the pancreatic tail, and a diagnosis of solitary pancreatic insulinoma was comfirmed. The position of the tumor was comfirmed by intraoperative ultrasonography, and laparoscopic resection of the pancreatic tail was per-formed. The operation time was 197 minutes, and blood loss was 10ml.
