尿膜管遺残症に対する腹腔鏡下切除術

Laparoscopic excision for an urachal remnant

　腹腔鏡下根治術を施行した尿膜管遺残症の1例を報告する．症例は24歳，男性．下腹部痛を主訴に来院し，身体所見や腹部CT検査にて臍下から正中腹壁に膿瘍を認めることから感染性尿膜管遺残症の診断となった．切開排膿術および抗生剤による治療後，待機的に腹腔鏡下切除術を施行した．手術は気腹法，4ポートにて行い，膀胱頂部から臍側へ剥離を進め，臍部直下にて瘻孔を切離，摘出した．手術時間は130分で，出血はごく少量，合併症は認めなかった．術後経過は順調で，6病日に退院となった．検索する限りでは，尿膜管遺残症に対する腹腔鏡下手術の本邦報告例はわれわれの報告を含め39例である．腹腔鏡下手術手技を中心に文献的考察を加えて報告する．

We herein report a case of urachal remnant resected using the laparoscopic technique. A 23-year-old man was seen at our hospital because of lower abdominal pain. Physical examination and abdominal CT revealed ab-scess formation in the lower abdomen. With a diagnosis of infected urachal remnant, incisional drainage and an-tibiotics treatment was performed. Laparoscopic resection of urachal remnant was performed successfully after conservative therapy. Using a four-port method with pneumoperitoneum, the umbilical urachal remnant was di-vided from the bladder and resected under the umbilicus.
