腹腔鏡下盲腸部分切除術を施行した虫垂粘液嚢腫の4例

Four cases of mucocele of the appendix on which laparoscopic partial resection of the cecum was performed

　虫垂粘液嚢腫に対し腹腔鏡下盲腸部分切除術を4例に行った．全例大腸内視鏡で盲腸内腔に粘膜下腫瘍様病変を認めた．症例4は，胆嚢胆管結石を合併していたため，腹腔鏡下経胆嚢管的胆道鏡下切石術を同時に行った．いずれも虫垂は緊満し黄白色ゼリー状の内溶液を認めた．術後の病理組織学的検索では，全例良性で悪性の所見は認められなかった．術後平均入院期間は9.3日であった．虫垂粘液嚢腫は術前に良・悪性の鑑別は困難であるため治療法の選択に苦慮することがあり，組織学的に良性でも半数以上に回盲部切除以上が選択されている．術前・術中に明らかな悪性所見がない場合は，腹腔鏡下盲腸部分切除術を第一選択にしてよいと思われる．

We performed laparoscopic partial resection of the cecum on 4 cases of mucocele of the appendix.

Colonoscopy showed a submucosal tumor in the cecum in all cases. Case 4 was a combination of mucocele and common bile duct stones, and we perfomed synchronous laparoscopic partial resection of cecum and transcystic cholangioscopic lithotomy. In each case, the appendix was expanded and filled with yellowish white jellied mucin. Postoperative pathologic examination showed that the mucoceles in all cases were benign tumors. The mean postoperative hospital stay was 9.3 days.
