急性胆嚢炎に対するPTGBD後の腹腔鏡下手術

Laparoscopic surgery after percutaneous transhepatic gallbladder drainage for acute cholecystitis

　急性胆嚢炎に対し，待機的に腹腔鏡下手術を行った82例を対象に，術前に経皮経肝胆嚢ドレナージ（percutaneous transhepatic gallbladder drainage：PTGBD），あるいは経皮経肝胆嚢穿刺吸引（percutaneous transhepatic gallbladder aspiration：PTGBA）を施行した27例（PTGBD（A）群）と非施行例55例（非PTGBD（A）群）の治療成績について比較検討した．平均年齢はPTGBD（A）群：61歳，非PTGBD（A）群：54歳で，性別，腹部手術既往，術前併存疾患には有意差は認めなかった．PTGBD（A）までの平均期間は4日，PTGBD（A）群，非PTGBD（A）群それぞれの発症から手術までの平均期間は27.4日，24.5日，平均手術時間は174分，184分，平均出血量は115ml,143ml，開腹移行は4例（15.4％），9例（15.8％）で，術中および術後合併症，術後平均入院期間，総入院期間にも有意差は認められなかった．以上より，重症胆嚢炎にPTGBD（A）が施行されていたにもかかわらず，PTGBD（A）群は非施行例に比べ手術成績は遜色なかった．急性胆嚢炎例に対し，待機的に腹腔鏡下手術を安全に施行するためには，重症例にPTGBD（A）を施行することが有用である．

We experienced 82 patients with scheduled laparoscopic cholecystectomy for acute cholecystitis. Out of them, twenty-seven patients (32.9%) with preoperative percutaneous transhepatic gallbladder drainage or aspiration (PTGBD (A) group) were compared with 55 patients without PTGBD (A) (Non-PTGBD (A) group). The mean ages of PTGBD (A) and Non-PTGBD (A) groups were 61 and 54 years. There were no significant differences in gender, previous abdominal surgery and preoperative combined diseases.
