Hand-assisted laparoscopic surgery（HALS）下に脾臓摘出術を行った脾炎症性偽腫瘍の1例

Resection of inflammatory pseudotumor of the spleen by hand-assisted laparoscopic surgery

　患者は64歳，女性．左上腹部痛を認め，脾腫瘍の診断で当院を紹介された．CT検査で脾に6cm大の腫瘍を認め，超音波内視鏡下のfine-needle aspiration biopsyを施行した．脾炎症性偽腫瘍の診断でhand-assisted laparoscopic surgery（HALS）を用いて脾摘出術を施行した．術後経過は良好であった．脾腫瘍の診断においては，まれな炎症性偽腫瘍も念頭に置くべきものと考えた．また脾臓を粉砕せずに摘出する必要のある脾腫瘍ではHALSを用いた腹腔鏡補助下の手術が操作性，安全性の面で適していると考えられた．

A 67-year-old woman complaining of upper abdominal pain was admitted to our hospital with a diagnosis of splenic tumor. Abdominal CT scan showed the splenic tumor that measured 6 cm in diameter. A fine-needle as-piration biopsy was performed under the endoscopic ultrasonography. Laparoscopy-assisted splenectomy using hand-assisted method was performed under the clinical diagnosis of inflammatory pseudotumor of the spleen. The post-operative course was favorable.
