胸部食道癌手術におけるHALS（hand-assisted laparoscopic surgery）の有用性

Usefulness of hand-assisted laparoscopic surgery (HALS) in radical esophagectomy for thoracic esophageal cancer

　食道癌手術において，近年，手術侵襲軽減を目的に内視鏡下手術が普及しつつある．われわれは胸部食道癌症例に対して，HALS併用右開胸開腹食道切除再建術を2000年に開始した．そこでHALSが手術侵襲軽減に貢献するかどうかを検証する目的で，HALSを施行した33例と同時期の開腹にて手術された33例の2群間で手術侵襲程度をretrospectiveに比較・検討した．手術時間，郭清リンパ節個数は両群間で差はなかったが，全出血量，腹部出血量はHALS群で有意に少なかった．また白血球，CRPの推移は両群間で差を認めなかったが，SIRS期間はHALS群で有意に短縮していた．上記の結果より，胸部食道癌手術においてHALSは根治性を損なうことなく手術侵襲の軽減につながることが示唆された．HALSは食道癌手術における標準術式として有用と考えられた．

Innovated endoscopic techniques have recently been introduced to reduce surgical invasiveness in the field of esophageal surgery. To evaluate the potential benefits of hand-assisted laparoscopic surgery (HALS) in radical esophagectomy for patients with carcinoma of the thoracic esophagus, surgical invasiveness in patients who received HALS (n=33) or conventional laparotomy (n=33) was compared retrospectively. No difference was seen in the background of the patients, operation time of abdominal procedure and the number of dissected lymph nodes between the 2 groups.
