脳室腹腔内シャントに合併した腹腔内仮性嚢胞に対して腹腔鏡下手術を施行した1例

Laparoscopically treated abdominal pseudocyst caused by ventriculo-peritoneal shunt:A case report

　脳室腹腔シャントの合併症として，まれに腹腔側のチューブ先端に仮性嚢胞を発症することが報告されている．今回，腹腔鏡下に根治術を施行した仮性嚢胞の1例を経験したので報告する．症例は56歳の男性，既往歴として20歳時に脳室腹腔シャント術を受けていた．上腹部痛，腹部膨満感があり，腹部CT検査を受けたところ巨大な肝嚢胞を指摘され，外科に紹介された，手術所見では，右横隔膜下に大網に被覆された巨大な嚢胞がみられ，腹腔鏡下に開窓した．シャントチューブは嚢胞内に迷入しており，その先端からは無色透明な髄液の流出がみられた．嚢胞壁は組織学的には炎症を伴う線維性被膜であり，脳室腹腔シャントに起因する腹腔内仮性嚢胞と診断した．

As a complication of ventriculo-peritoneal shunt, abdominal pseudocysts have been reported in rare occasion. This paper describes a patient with abdominal pseudocyst caused by ventriculo-peritoneal shunt that was re-moved laparoscopically. A 56-year-old male who had received a ventriculo-peritoneal shunt procedure in the past was admitted to our hospital complaining of upper abdominal pain and distension. Computed tomography revealed a huge cyst in the liver and the patient was reterred to the surgical department.
