後腹膜腔鏡下ドナー腎摘出術

　当院では，1975年以降687例の生体腎移植を施行している．2003年以降，完全内視鏡下による後腹膜腔鏡下ドナー腎摘出術を61例に施行した．成績は平均手術時間314±94.5分，平均出血量77.4±94.5 mlであった．移植腎機能に問題はなかった．合併症として気胸を3例に認めた．肺梗塞，大動脈解離をそれぞれ1例認めた．6例で小開創して腎臓を摘出した．手術時間は13例以降比較的安定した．後腹膜腔鏡下ドナー腎摘出術は，術後のQOLを高めるうえ，比較的安全であった．ラーニングカーブがあるため，手術症例の比較的多い施設では，今後，ドナー腎摘出術の主流になる可能性も考えられた．

We have performed kidney transplantation in 687 patients from 1975 to 2003. Sixty-one of the patients under-went strictly-defined retroperitoneoscopic live donor nephrectomy. The mean operation time was 314±94.5 minutes, and the average of blood loss was 77.4±141 ml. The average of warm ischemic time was 3.8±1.41 min-utes. Post surgical complications were as follows: mild pneumothorax in 3 patients (4.9%), pulmonary infarction in 1 patient (1.6%) and dissection of the aorta in 1 patient (1.6%).
