魚骨迷入が原因となった急性虫垂炎の腹腔鏡下手術施行の1例

A case of acute appendicitis caused by fish bone

　患者は57歳，男性．腹痛を主訴に来院した．腹部超音波検査とCT検査にて虫垂内に糞石もしくは結石を伴う急性虫垂炎と診断し，腹腔鏡下虫垂切除術を行った．術中所見では虫垂の先端は膿苔を形成していたが穿孔しておらず，虫垂根部には炎症の波及がなく結紮・切離が可能であった．摘出標本の内部から膿汁とともに約2cmの魚骨がみられ，組織学的に蜂窩織炎性虫垂炎と診断された．今回の症例は手術所見で虫垂の先端に膿苔を形成していたことより，魚骨の先端から炎症が波及し虫垂炎を惹起したと思われた．また腹腔鏡下手術により術中虫垂の全容が把握できたことは，安全に手術を施行するにあたり重要なことと思われた．

We report a rare case of acute appendicitis that was probably caused by a fish bone. A-57-year-old man suf-fered from abdominal pain and was admitted to the hospital. Acute appendicitis was diagnosed by physical ex-amination, ultrasonography and abdominal CT. A high-density area and a high echoic area in his appendix were detected with CT and ultrasonography, respectively. Laparoscopic appendectomy was performed. The postop-erative pathological diagnosis was phlegmonous appendicitis, and a fish bone of 2 cm in length was found in the excised appendix lumen.
