腹腔鏡下に摘出し得た腹腔内異物の1例―本邦報告32例の検討とともに―

Laparoscopic removal of a foreign body（a piece of wire）from the abdominal cavity : a case report and review of thirty two cases in Japan

症例は45歳，女性．心窩部痛を主訴として来院した．腹部X線検査にて第四腰椎左側に針状の異物を認めた．腹部CT検査では胃壁を貫通する異物が疑われた．上部内視鏡検査を施行したが，異物は発見できなかった．腹腔内異物と診断し，腹腔鏡下に観察したところ，胃前庭部前壁より突出する金属片を認めた．異物を鉗子にて把持し摘出した．抜去部からの出血，胃液流出は認めなかったので，胃壁の縫合などの追加処置は行わなかった．異物は長さ3cmほどの螺旋状の針金であった．術後は胃管による減圧とプロトンポンプ阻害薬投与を行った．経過は良好で術後8日目に退院した．腹腔鏡下異物摘出術の1例として，本邦報告32例の考察とともに報告する．

　Laparoscopic removal of a foreign body（a piece of wire）was successfully performed. A 45-year-old woman was admitted to our hospital because of an upper abdominal pain. An abdominal X-ray and CT scan revealed a piece of wire penetrating the gastric wall into the abdominal cavity. Laparoscopic retrieval was performed with the 2-port method（umbilical port for laparoscopy and upper abdominal port for forceps）. We found the wire sticking out of the gastric wall and removed it easily with laparoscopic forceps. It took only 19 minutes to finish the operation. The patient had an uneventful postoperative course and was discharged on the eighth day after the operation.
