予防措置にもかかわらず肺塞栓症を発症した腹腔鏡下直腸切除術の1例

A case of pulmonary embolism following laparoscopic rectal surgery with thromboembolism prophylaxis

　患者は63歳，女性．2004年2月23日，直腸癌に対して腹腔鏡補助下低位前方切除術（D1郭清）を行った．肺塞栓症予防のため，術中，術後に間欠的下肢圧迫装置，弾性ストッキングを使用したが，術後2日目に肺塞栓症を発症した．現在，ガイドラインで推奨されている予防措置は，このように術後の肺塞栓症を100％予防しうるものではない，腹腔鏡下手術後の肺塞栓症を減らすためには，腹腔鏡下手術が肺塞栓症の危険因子であることを認識し，ガイドラインに準拠した予防法を徹底することが重要で，さらに手術時間の短縮や気腹の間欠的開放など手術操作に関連した血栓予防に努める必要がある．

A 63-year-old woman with rectal cancer underwent laparoscopy assisted low anterior resection on February 28th 2004. Although an intermittent pneumatic compression device and elastic stockings were used for thromboembolism prophylaxis during the operation, she developed pulmonary embolism on the second post op-erative day. It may be suggesed that prophylactic measures recommended in the clinical guideline could not prevent pulmonary embolism perfectly.
