腹腔鏡下手術にて切除した肝副葉茎捻転の1例

Laparoscopic operation for symptomatic accessory lobe of the liver

　肝副葉に対して腹腔鏡下摘出術を施行した1例を経験した．患者は27歳，女性．急性腹症にて紹介となった．上部内視鏡で十二指腸が外部より圧迫され，腹部エコー，腹部CTにて，胆嚢の左側に5cmの腫瘤を認めた．腹痛が強く腹腔鏡下に緊急手術を施行した．手術にて肝円索と交通のある肝副葉の茎捻転であった．肝副葉はきわめてまれな疾患であるが，胆嚢周辺に不明な腫瘤を認める急性腹症の鑑別診断の一つとして念頭に置くことが必要と思われる，また手術は腹腔鏡下手術が第一選択だと考える．

We report a case of laparoscopic resection for accessory lobe of the liver. A 27-year-old woman complained of acute abdominal pain. An emergent endoscopic examination revealed that the duodenum was pressed from the outside. Abdominal ultrasonography and abdominal CT revealed a mass lesion of 5 cm, which was next to the gallbladder. Abdominal pain was so severe that we performed an emergency operation with laparoscopy. We found an accessory lobe of the liver upon operation.
