逆流性食道炎に対する腹腔鏡下手術

Laparoscopic Surgery for Reflux Esophagitis

　逆流性食道炎は，わが国において今後増加が予想される疾患であるが，プロトンポンプ阻害薬とともに逆流防止手術が有用な治療である．この逆流防止手術は腹腔鏡下手術へ移行してきているが，術式としては主にNissen法とToupet法が用いられている．これまで腹腔鏡下にNissen法を21例，Toupet法を16例に施行した．手術時間は105〜315分で，初期症例，肥満例で延長している．開腹移行例はなかったが，Nissen噴門形成術後の持続する嚥下困難症例と十二指腸潰瘍狭窄合併例に対するJaboulay胃十二指腸吻合部の狭窄例に対し再手術を必要とし，術後再発例は1例に認められている．多くは一過性の嚥下困難を呈するが，術後の満足度は高く，今後の普及が期待される．

The incidence of reflux esophagitis is increasing in Japan. Proton pump inhibitors and surgical fundoplica-tions are known to be effective for the treatment of this condition. Fundoplications for gastro-esophageal reflux have been performed laparoscopically, and the Nissen and Toupet procedures have become popular as laparoscopic surgical techniques. We have performed 21 Nissen and 16 Toupet fundoplications for patients with gastro-esophageal reflux. The operation time ranged from 105 to 315 minutes, and was longer in earlier cases and obese patients.
