　われわれは，超音波凝固切開装置や血管クリップを用いずに，LigaSure Atlas^TMのみを用いて腹腔鏡下大腸全摘術を潰瘍性大腸炎患者8例に施行した．ポートの位置，数，手順などは従来の方法と共通で，主要血管（回結腸，中結腸，下腸間膜動静脈）はLigaSure Atlas^TMを用いて一括して切離した．開腹手術へ移行した症例は認めなかった．術中および術後出血は認めなかった．手術時間は従来法に比べて約40分短縮した，また，コストは従来法に比べて，1症例あたり42,000円安価であった．LigaSure Atlas^TMは，クリップレス腹腔鏡下大腸全摘術において安全に使用可能で，cost-effectiveであることが示された．

We performed clipless laparoscopic restorative proctocolectomy in eight patients with ulcerative colitis using LigaSure Atlas^TM without ultrasonic coagulating shears or hemoclips. The number and the site of trocars and the basic procedure were the same as the conventional method. The main vessels (ileocolic, middle colic, and the in-ferior mesenteric arteries and veins) were securely divided by LigaSure Atlas^TM . There were no conversions to the open procedure. There were no intraoperative or postoperative hemorrhagic complications.
