　【目的】Vessel sealing system LigaSure^TM（以下，VSS：タイコヘルスケア　ジャパン社）の肝・胆・膵領域手術への応用の可能性について基礎的研究を行った．【方法】（1）摘出したヒトの胆嚢で胆嚢管をVSSで変性，融合させ，閉鎖された胆嚢管断端の耐圧強度を測定した．（2）イヌを用い，VSSのみを用いて肝切除，膵切除，胆嚢摘出を行った，1週間後に屠殺して再開腹し，各断端の所見を検討した．（3）臨床の肝臓，膵臓の切除材料を使って切除断端の閉鎖を行った．【結果】（1）胆嚢管断端は平均180mmHg以上の圧に耐久した。（2）VSSで処理した各断端部はいずれも閉鎖していた．（3）臨床材料でも同様な結果であった．【結語】さらなる検討が必要であるが，VSSは肝・胆・膵領域の手術にも有効と思われた．

[Introduction] Vessel sealing system (VSS, Liga Sure^TM, Valleylab, USA) is a new operating device. This hemostasis product works by fusing the collagen in vessel walls to create a permanent seal. We performed basic and clinical research using VSS for hepato-biliary-pancreatic surgery. [Methods] 1) We examined the burst resistant pressure of cystic duct cut by using VSS in five excised human gallbladders. 2) In canine model, we performed hepatectomy, pancreatectomy, and cholecystectomy, using only VSS. Macroscopic and histopathological examinations were performed.
