前胸部アプローチ法による内視鏡下甲状腺手術

Endoscopic Thyroidectomy by the Precordial Approach

　甲状腺良性疾患は女性に多く，特にバセドウ病は若年者に多いことから，わが国では整容性を重視し，頸部に予術創を残さない種々のアプローチ法による内視鏡下甲状腺手術が考案されてきた．筆者らは，バセドウ病への応用も考え，良好な視野が確保できる前胸部アプローチ法を選択している．本術式は，前胸部から前頸部に手術操作腔を作製する必要があるが，内視鏡の拡大視による精度の高い手術が実施でき，また，患者の整容的な満足度が高く，術後QOL（quality of life）の向上に有用な術式である．

Benign thyroid diseases are more frequent in females and Graves' disease is especially more common in younger patients. Therefore, in Japan, various approaches have been studied in order not to leave any undesir-able scar on the neck and as a result, endoscopic thyroidectomy was developed. We have selected endoscopic thyroidectomy based on the precordial approach method which makes it possible to provide fine magnification of the thyroid anatomy.
