There are two roles of surgery in advanced gynecologic malignancies. One is surgical staging, which is helpful in individualizing treatment such as deciding the radiation field or neccessity of chemotherapy. The second is a cytoreductive role which enhances the effectiveness of radiation or chemotherapy. In advanced cases for whom curative resection is impossible, it is unreasonable to perform an invasive laparotomy which might delay primary treatment (radiation therapy or chemotherapy). In contrast, endoscopic surgery offers the patient a number of advantages.
