進行性腎癌に対する腹腔鏡下根治的腎摘除術—その適応と限界

Laparoscopic Radical Nephrectomy for Advanced Renal Cancer

　腎癌に対する腹腔鏡下手術の可能性と限界について検討した．1997年9月から2001年12月までに腎癌に対して腹腔鏡下手術を行った37例のうち，進行性腎癌として臨床診断T2以上の7例を対象とした．腫瘍茎7cm以上の6例はハンドアシストにて，腎静脈内腫瘍塞栓を有する1例はmorcellation併用にて，経腹腔的前方到達法による腹腔鏡下根治的腎摘除術を行った．1例は強度の癒着により開放手術へ移行した．開放手術移行症例を除いた平均手術時間は251分，平均出血量は308mlであった，1例に術後無気肺を認めた．2例は遠隔転移にて癌死した．T2以上の腎癌に対しても腹腔鏡下根治的腎摘除術は安全で有用な手術法である．

The availability of laparoscopic radical nephrectomy for patients with advanced renal cancer is evaluated. Seven patients with advanced renal cancer more than clinical stage T2 were treated with laparoscopic radical nephrectomy via transperitoneal anterior approach from September 1997 to December 2001. Six patients who had renal cancer more than 7 cm in diameter were treated with hand-assisted procedure. One patient who had renal cancer with renal vein thrombus was treated with pure laparoscopic surgery by using morcellation in combination. One patient needed conversion to open surgery due to severe adhesion.
