Endoscopic Retroperitoneal Lynphadenectomy: Multiple Vantage Points Approach

　婦人科悪性腫瘍において後腹膜リンパ節郭清は，ステージングあるいは治療的切除のためにも重要な手技の1つである．近年の手術機器の進歩，手術手法の普及により婦人科疾患の多くが内視鏡下に行われるようになった．この内視鏡下手術の利点である低侵襲性は，本来ラディカルな癌手術に導入してこそ本領が発揮できるものと考え，子宮癌と卵巣癌に対して内視鏡下による後腹膜リンパ節郭清を開始し，これまで手技・手法において創意・工夫をはかつてきた．本法は，視覚的な切除の完全性とリンパ節の回収数からみても，その手術内容は従来の開腹法に引けを取らないものと考える．一方，術後の回復は速やかで，出血量も少量，イレウスなどの腸管合併症は激減した．

In gynecologic malignancies retroperitoneal lymphadenectomy is one of the most important strategies for both staging and treatment. Recently, because of the development of new instruments and operative techniques, most of the benign tumors have been operated endoscopically. The introduction of endoscopic surgery in the case of gynecologic malignancies has been delayed. Endoscopic surgery, being less invasive, plays an important role in the case of radical surgery for gynecologic malignancies. Therefore, we have introduced endoscopic sur-gery for the treatment of cancer of the uterus and ovaries.
