後腹膜鏡下傍大動脈・骨盤リンパ節郭清術

Retroperitoneoscopic Para-aortic and Pelvic Lymph Node Dissection

　後腹膜リンパ節郭清術は婦人科悪性腫瘍に対する治療であるとともに，術後の治療の個別化のためのステージングとしても重要な手技である．しかし，従来の開腹手術ではきわめて大きな術創を必要とし，腸管に対する影響が大きく，術後の回復，また美容的見地からも課題が残されていた．そこでわれわれは，きわめて侵襲の小さい後腹膜アプローチ法を開発して，後腹膜鏡による全後腹膜リンパ節郭清術を考案し確立させた．本法は出血が少なく，術後の回復はきわめて早く，腸管麻痺や腸閉塞などの術後の合併症も少ない．また，死角への対処法として血管テープによる大動静脈の牽引・移動，多視点からの観察，30度光学視管の使用などの創意工夫により，十分な後腹膜リンパ節郭清が可能であると考える．

The retroperitoneal lymphadenectomy is a mainstay for therapy and staging, tailoring the post-operative therapy for individual cases, for gynecologic malignancies. The traditional laparotomy has been problematic be-cause of the resulting adverse affects to the bowel, delayed recovery and, from a cosmetic point of view, the ne-cessity of a large incision. Therefore, we have developed a less invasive retroperitoneal approach and established a method of simultaneous para-aortic and pelvic lymphadenectomies with the aid of the retroperitoneoscope.
