胃内発育型平滑筋腫に対する漿膜筋層切開を付加した腹腔鏡下胃局所切除術

Laparoscopic Local Gastric Resection with Seromyotomy for Intragastric Leiomyoma

　胃粘膜下腫瘍に対して腹腔鏡下胃部分切除が積極に行われるようになってきた．しかし，胃内発育型粘膜下腫瘍では腫瘍径が大きい場合，胃壁外から部分切除を行うと腫瘍部の把持が困難なだけでなく，広範囲の切除を要するため，残胃の変形が強くなる．今回，われわれは胃穹窿部後壁側と胃角部前壁側に存在し増大傾向を示す胃内発育型平滑筋腫の2症例を経験した．切除範囲を必要十分な大きさとし，切除後の残胃の変形を最小限にする目的で腫瘍周囲の漿膜筋層切開を加えたのち，胃内腔に突出した腫瘍を胃壁外に誘導し，腹腔鏡下胃局所切除術を施行した，本術式は胃内発育型粘膜下腫瘍において，ほかの術式に比べ有用と思われたので報告する．

Laparoscopic local gastric resection has recently been performed actively for gastric submucosal tumor. However. in case of a large intragastric submucosal tumor. it is difficult to grasp and lift the tumor steadily, and to prevent the deformity of residual stomach by excising an unnecessary larger margin of normal tissue. We ex-perienced 2 cases of enlarged intragastric leiomyoma located on the posterior aspect of the fundus and anterior aspect of the angle of the stomach.
