尿膜管臍瘻に対する腹腔鏡下摘出術

Laparoscopic Excision of Umbilical Urachal Sinus

　われわれは尿膜管臍瘻症例に対して，腹腔鏡下に摘出術を施行したので報告する．腹腔鏡下尿膜管臍瘻摘出症例は4例で，いずれも臍炎を主訴とし，平均年齢は24.3歳であった．3本のトロッカーを腹腔内に挿入し，膀胱近傍から臍にかけて腹腔鏡下に尿膜管臍瘻を摘出した．手術時間は平均75.5分で，病理学的にはいずれも良性の尿膜管遺残であった．術後の経過は良好で，術後平均入院期間は6.3日，また10日以内に社会復帰が可能であった．術後平均7.5か月間が経過したが，術中および術後に合併症は認めなかった，尿膜管臍瘻に対する腹腔鏡下手術は手術侵襲が小さく，整容上も優れていることより大変有用であると思われた．

We experienced 4 patients of umbilical urachal sinus which underwent successful laparoscopic excision, Four patients with a mean age of 24.3 years who complained of omphalitis underwent laparoscopic excision of the um-bilical urachal sinus. Using two 10 mm and one 5 mm ports, the umbilical urachal sinus and medial umbilical liga-ment were divided from the bladder cephalad to the umbilicus. The specimen, which included the umbilical urachal sinus and medial umbilical ligament, was then separated from the umbilicus and removed intact. All 4 patients were operated successfully.
