腹腔鏡下にて摘出し得た尿膜管臍瘻の1例

A Case of Laparoscopic Excision of an Umbilical Urachal Sinus

　われわれは尿膜管臍瘻に対し，腹腔鏡下にて摘出術を施行したので報告する．症例は25歳の女性，主訴は臍よりの膿汁分泌であった．腹部CTにて尿膜管臍瘻と診断され，腹腔鏡下摘出術を施行した．トロッカーは右下腹部，臍下部やや右側，右側腹部にそれぞれ挿入した．腹腔鏡にて臍直下より正中尾側に長さ約4cmの嚢胞性病変を認めた．嚢胞頭側は臍直下にて結紮，尿膜管尾側断端は吸収クリップにて二重に結紮し，嚢胞を剥離・切離し腹腔内から摘出した．術後，順調に経過し再発や臍部の変形などを認めていない．本術式は従来の術式と比べて，患者への侵襲・美容上の問題といった点で優れており，尿膜管疾患に対する有用な術式であると考えられる．

We experienced a case of umbilical urachal sinus which underwent successful laparoscopic excision. A25-year -old woman was seen at the hospital because of pus discharge from the umbilicus. An umbilical urachal sinus was diagnosed by computerized tomography of the abdomen and laparoscopic excision was performed. In the operational procedure, three trocars were inserted into the peritoneal cavity ( A 10 mm trocar at the right lower abdomen for laparoscope, a 5 mm trocar at right flank and a 10 mm trocar at the middle lower abdomen) .
