乳腺内視鏡手術と麻酔管理

Endoscopic Mastectomy and Its Anesthetic Management

　外科手術が発展するなかで，麻酔管理の進歩と種々の手術器械の工夫が実現されてきたが，その土壌のうえに開花したのが現代の内視鏡外科手術である．乳腺内視鏡手術は，各種内視鏡下手術の経験をもとに，現代の外科治療に求められる新たな命題「低侵襲性」と「整容性」を追求して登場した．麻酔科医からみると，内視鏡下手術全般が必ずしも低侵襲とはいえないなかで，乳腺内視鏡手術はほぼ満足できる低侵襲性を実現し，いまや日帰り手術の格好の対象となっている．われわれの施設では，現在新病院を建設中で2002年の4月に開院予定であるが，そこで日常的に展開されるであろう日帰り手術を展望して，乳腺内視鏡手術をモデルケースとしたシステム構築を進めている．

With the advancement of anesthesiology and various operative tools, surgery has established its status and endoscopic surgery is one of the modern fruit of its development in this respect. Endoscopic mastectomy en-tered this field to pursue less aggressiveness and more aesthetic appearance and has now become a “student of honor”. From the anesthetic point of view. endoscopic mastectomy is less aggressive to patients than other laparoscopic or thoracoscopic surgeries.
