Indications and limitations of endoscopy assisted surgery for breast cancer

　乳癌に対する内視鏡手術は，癌の根治性を落とすことなく乳房の整容性を追求した手術である．それゆえ乳房温存術がそのよい適応となる．この場合は，直視下の乳房温存術の適応と変わらないと考えている．また，乳房切除術（乳腺全摘）でも乳房の再建を一期的に行う場合は内視鏡手術の適応である．乳癌の高度な胸筋・腋窩リンパ節浸潤あるいは皮膚浸潤が見られる場合は，術後の乳房の整容性を維持できない術式をとらざるを得ないことから適応外と考えている．乳腺内視鏡手術は，どんな乳癌に対してもまた手術のはじめから最後まで内視鏡を使用する必要はなく，癌の根治性を優先したうえで，術後の乳房の整容性を追求できる術式が選択できる場合にのみ適応となり，乳癌手術の一連のなかでの手術手技の一つであると考えるべきである．

Endoscopy assisted surgery for breast cancer is an operative technique, for both curative and cosmetic pur-poses. The best indication of this technique is breast-conserving surgery (BCS). The indications of BCS with endoscopy-assisted surgery are same as those of BCS without endoscopy. On the other hand, skin sparing mas-tectomy or total glandectomy of the breast with concomitant reconstruction is also an indication for endoscopic procedure.
