 Recently the number of geriatric subjects undergoing anesthesia and surgery is increasing. The term “urgent surgery” is defined in ACC/AHA guideline, as the surgical treatment necessary within 6 to 24 hours after diagnosis. Proximal femoral fracture typically occurs in geriatric subjects and its treatment is usually classified as urgent surgery. Additionally, Japanese government provides additional incentives to surgically correct proximal femoral fracture in patients older than 75 years. Such policy may further increase the number of urgent surgeries and appropriate knowledge is imperative to provide adequate perioperative care to these subjects.

 In the first part of this thematic series, preoperative evaluation of cardiac comorbidities, frailty and sarcopenia, respiratory conditions are discussed by the experts. This report discusses, pharmacokinetic and pharmacodynamic of anesthetic drugs in the elderly, the strategies to the selection of anesthetic methods and timing of surgery, the etiology and prevention of postoperative delirium/cognitive dysfunction. Lastly, technical tips and pitfalls how to provide neuraxial block in elderly patients are provided. These articles may provide relevant information how to provide better anesthesia care in the elderly who undergo urgent surgery.
