KPコットン®を利用した吊り上げ式ハンドアシスト腹腔鏡下結腸切除

A Hand Assisted Laparoscopic Surgery for Advanced Colon Cancer using KP-Cotton® without Pneumoperitoneum

　最近，腹腔鏡補助下腸切除にhand assisted laparoscopic surgery（以下，HALS）が試みられるようになってきた．ところが，吊り上げ式では良好な視野が得られないとの理由から，HALSにおいてももっぱら気腹法が用いられている．そこでわれわれは，吊り上げ式で行う際に38×30cm大のガーゼ（KPコットン^®）を使って小腸を術野から圧排することにより，気腹法による手技の煩雑さを解消した．その結果，得られた利点は以下の通りである．①過度な体位変換をせずに，良好な視野が得られる．②小腸は完全に術野から隔離，保護され，電気メスなどによる小腸の思わぬ損傷も回避できる．③一般手術器具の併用で，中枢側のリンパ節郭清も確実に行うことができる．④cost benefitの面からも有利である．以上から，今後，結腸癌に対する一術式として選択されうるのではないかと考えられた．

Hand Assisted Laparoscopic Surgery (HALS) has become a feasible technique for colon cancer because of tactile sensation and free movement of the surgeon's left hand in the abdominal space. However, it is currently performed with pneumoperitoneum to gain a wide view. To avoid complicated handling of pneumoperitoneum, we used a big gauze sheet (KP-cotton^®) measuring 38×30cm. In spite of abdominal wall lifting application, this technique enabled good view of operation field without excessive head down position.
