IVs神経芽腫に対する後腹膜アプローチによる内視鏡下生検

Retroperitoneoscopic Tumor Biopsy for Stage IVs Neuroblastoma

　これまで神経芽腫に対する内視鏡下手術はマススクリーニング例を対象として比較的小さな腫瘍が対象とされてきた．一方，神経芽腫の治療にとって腫瘍組織の生物学的特性を知ることは化学療法regimenの選択と予後の判定にとって必須となっている．しかし，進行例に対する腹腔鏡下生検は出血，視野の確保ならびに腫瘍細胞の腹腔内播種の危険から行われていない．後腹膜アプローチによる腫瘍生検は以上の内視鏡下生検上の隘路を克服することが可能であり，IVs期神経芽腫（マス例）に対して安全に施行することができる．手技の詳細を中心に概説する．

Minimally invasive endosurgical tumor biopsy with a retroperitoneal approach was successfully done in an 8 month-old boy with stage IVs neuroblastoma. The retroperitoneal space was entered with a small incision on the left posterior axillar line and the working area was created using a balloon dilator. The adrenal tumor was easily exposed at its anterolateral plane and the biopsy specimen was taken after clipping on the vascular supply. The approach gave a sufficient operative view from the adrenal gland down to the ureter, including the whole kidney.
