縦隔腫瘍に対する胸腔鏡下手術

Thoracoscopic Surgery for Mediastinal Tumors in Children

　小児における胸腔鏡下手術は未だ広く普及するには至っておらず，現時点で腫瘍性病変に対する明確な手術適応は存在しない．本稿では胸腔内腫瘍に対する胸腔鏡下アプローチの1つである胸腺原発成熟奇形腫に対する胸腔鏡下摘除術を概説する．また小児における片側換気法，狭い胸腔内での鉗子の操作性や標本の体外摘出などの問題に対するわれわれの現在の取り組みを紹介する．併せて，悪性腫瘍に対する胸腔鏡下手術の適応拡大の可能性についても考察を加える．

Thoracoscopic surgery for intra-thoracic tumors has not gained wide clinical acceptance in the pediatric population. We herein describe our current technique of thoracoscopic surgery for mediastinal mature teratomas in children. Technical problems and limitations in one-lung ventilation, instrumentation and specimen delivery are reviewed. A potential indication for intra-thoracic malignant tumors is also discussed.
