超音波凝固切開装置による肝実質の離断（開腹手術）

Hepatectomy Using the Ultrasonically Activated Scalpel (Laparotomy)

　超音波凝固切開装置は臓器の把持，剥離，切開，凝固など多岐の機能を併せもつため，開腹下の肝実質離断に使用している．留意点としては，（1）十分に超音波検査を行い予定離断面と脈管との位置関係を十分に検索し主要脈管周囲にはtattooingする，（2）肝の硬度に合わせてはじめに切開，凝固のバランスの調節を出力レベル，ブレードの形状などで行う，（3）離断面に適度なテンションを保ち視野を常に確保し，深部では振動子を上に向けて操作することが重要である．自験例の検討では，非硬変肝で葉切除などフラットで広い離断面を要する肝切除において，特に有効性が示唆された．今後は，ミストによる播種の問題，胆管に対する影響，操作性の改良について検討されねばならないと考えられる．

Since the ultrasonically activated scalpel has multiple functions such as grasping, dissecting, cutting and coagulating in a single instrument, it has been used for hepatectomies. It should be important to pay attention to considerate intraoperative ultrasonography to comprehend the correlation between the vessels and dissecting line and tattooing around the main vessels.
