A 54 year-old man had a history of intractable reflux esophagitis due to duodenal obstruction that was caused by recurrent duodenal ulcer and chronic cholecystitis. We underwent laparoscopic fundoplication (Toupet's pro-cedure). proximal selective vagotomy (PSV), pyloroplasty and cholecystectomy. Fundoplication and PSV were easy. but the operation required more than six hours because of the strong inflammation of the gall gladder. Thepost-operative course was unremarkable. However because of the scar stricture of the esophagus. early short of the meal and early discharge were not possible.
