Single-portデバイスを用いた自然気胸の胸腔鏡下手術

Video-assisted thoracoscopic surgery using a sponge-like single-port device for spontaneous pneumothorax

◆要旨：病変が限局している自然気胸に対し，スポンジ状single-port deviceであるSILSポート^®（コヴィディエン社）を用いた肺縫縮，肺部分切除を17例経験した．本法導入前は12mm径のトロッカー2本と5mm径トロッカー1本を用い3ポートで施行していたが，鉗子操作で肋間神経が圧挫されるため，術後疼痛の遷延が目立った．現在は，ソフトなスポンジ状（サーモプラスティックエラストマー）のSILSポート^®を使用することにより，術後肋間神経痛を最小限にすることができた．また，整容性も優れている．当科での本術式の工夫および手術成績を報告する．

We have performed pleural plication or partial pulmonary resection using the SILS Port^®―a sponge-like single-port device―for spontaneous pneumothorax with localized blebs or bullae in 17 cases. Originally, such procedures were carried out using two 12mm- trocars and one 5mm trocar, which resulted in significantly prolonged postoperative pain due to intercostal nerve injury induced by thoracoscopic surgery. In addition to facilitating single port surgery, the use of the soft sponge-like SILS Port^® has led to less postoperative intercostal neuralgia. This method of surgery is also superior in cosmesis.
