腹腔鏡下手術における体位固定の工夫―2枚のマジックベッドを用いた方法―

An improved patient immobilization method for laparoscopic surgery using a double magic-bed

◆要旨：腹腔鏡下手術において良好な術野確保のためには体位変換が不可欠である．術式によっては，頭低位，左右斜位を維持する際に体幹部・四肢にかかる圧や牽引力による神経障害・皮膚障害が問題となることがある．これらの障害を回避するため，体位固定の際に頭側，足側に2枚のマジックベッドを用い，体幹を十分に固定する方法を導入した．2008年から腹腔鏡下手術50例に本法を施行したが，神経障害，皮膚障害は認めておらず，安全で簡便な方法であると考える．現在，当科で行っているマジックベッドを2枚用いた体位固定の工夫について報告する．

　During laparoscopic surgery, it is sometimes necessary to change the position of the patient to obtain a clear operative field. When a severe head-down position or side rotation is required, it is important to avoid any neuropathy or skin injury resulting from compression and traction with an immobilization appliance. We have designed a safe and easy method for fixing the position of a patient using a double magic-bed. One of the magic-beds is used for fixing the upper part of the body, and the other for fixing the lower part of the body. Since 2008, we have applied this method to 50 patients, and none of them have suffered neuropathy or skin injury. Here we report the details of this device.
