胃内発育型GISTに対する腹腔鏡下手術の工夫

A technique of laparoscopic resection of gastric GIST with intra-luminal growth pattern

　胃内発育型GISTに対する腹腔鏡下手術における漿膜切開法について紹介した．腫瘍辺縁の漿膜に切開を置くことにより胃内発育型は胃外発育型様に変容し，容易にその根部のスネアリングが可能となる．スネアリング後は必要最小限の自動縫合器により安全確実に切離でき，腫瘍径に応じて必要かつ十分なサージカルマージンを確保することができる．この際，腫瘍の被膜を損傷しないことに注意すべきで，腫瘍辺縁の漿膜を慎重に切開し，すみやかにスネアリングとEndo-Pouch内への収納を行うことで腫瘍細胞の播種を防げるものと思われる．本法は胃内発育型GISTに対する腹腔鏡下手術の戦略の一つとして有用であると思われた．

The difficulties of laparoscopic surgery for gastrointestinal stromal tumor (GIST) of the stomach with an intra-luminal growth pattern are attributed to the difficulties in detecting the tumor location and in deciding the resection area based on laparoscopic information. Transillumination and serosal dissection methods were useful in these situations. Not to injure the tumor capsule, and to limit the serosal dissection enough for snaring the tumor with extra-luminal transformation are the key points in this method.
