In the case of laparoscopic proximal gastrectomy in which the lower esophagus should be transected, esophagogastrostomy using the gastric tube is performed at our institute. Gastric tube facilitates the reconstruction in the mediastinum with shorter operating time. Generally, postoperative reflux esophagitis is a serious problem after esophagogastrostomy. To solve this issue, we use no-knife endoscopic linear stapler for the anastomosis. The space between the staple lines is separated for 1.5cm to create the anastomotic lumen. By anastomosing the gastric tube to the dorsal part of the esophagus, the undissected part of the gastric tube, from the staple line, resembles the gastric fundus in shape. This new method using the no-knife endoscopic linear stapler is useful for the prevention of the reflux esophagitis after proximal gastrectomy.
