胃癌手術における腹腔鏡下デルタ吻合150例の臨床評価―術後アンケートから

Clinical results of Delta-shaped Anastomosis

◆要旨：筆者らは腹腔鏡下幽門側胃切除における体内Billroth I器械吻合（デルタ吻合）の手技を7年前に確立し，この方法の簡便性と安全性についてすでに報告した．今回は，患者アンケートをもとにした臨床評価を報告する．

　2001年5月～2008年2月までに150例に本術式を施行した．縫合不全は1例のみで，術後早期に絶食を要する胃内容停滞例を1例も認めず，術後早期の合併症は少なかったが，ダンピング症候群などの発生頻度は未調査であった．このため，これら150例を対象にアンケート調査を行ったところ，ダンピング症候群と診断されたのは，わずか1例のみと臨床評価も良好な結果であった．

　術後イレウスのなどの遠隔期合併症の発生もなく，デルタ吻合は完全腹腔鏡下に行えるBillroth I再建術として有用な吻合法の1つと考える．

　We developed a new method of intracorporeal gastroduodenostomy, named“Delta-shaped Anastomosis“, 7 years ago and have already reported our initial trials. In this paper, we explain the middle and long term results of this procedure. We performed this anastomosis in 150 gastric cancer patients. The mean time for the procedure was approximately 13 minutes. Only one patient developed minor leakage, while the others had no complication. Oral intake at early stage was not problematic, and no patient developed stasis. However, Dumping syndrome developed in one patient. These results indicate that the Delta-shaped Anastomosis is an ideal choice for reconstruction in laparoscopic distal gastrectomy.
