 We have performed 200 cases of totally laparoscopic gastrectomy from August 1999. In totally laparoscopic gastrectomy, minilaparotomy is only for taking out the resected specimen. Until April 2006, we chose the U-shaped skin incision around the umbilicus, followed by longitudinal incision of the fascia. However, large specimens were often difficult to remore. To solve this issue, we changed the procedure to dividing the umbilical depression at the mid line, and have already performed 48 cases. This procedure enables us to remove the large specimens easily and safely. Since this procedure produces cosmetically better results, we recommend this procedure in totally laparoscopic gastrectomy.
