◆要旨：1999年8月～2007年5月現在までに200例の完全腹腔鏡下胃切除術を施行した．完全腹腔鏡下胃切除術では切除から再建までのすべての操作を体内で行うため，最大の皮膚切開創は標本を体外へ摘出する創部となる．当初は臍周囲皮膚をU字に，筋膜を縦に切開する方法を用いていたが，手技的により簡便でかつ美容的に優れた方法を目指し，2006年4月からは臍そのものを縦切開する方法に変更し，2007年5月までに48例に施行した．結果は，大きな標本の摘出も比較的簡単で，創感染などの合併症もなく，創部の外観も良好であった．臍縦切開法は完全腹腔鏡下胃切除術における標本摘出に有用な方法と考える．
We have performed 200 cases of totally laparoscopic gastrectomy from August 1999. In totally laparoscopic gastrectomy, minilaparotomy is only for taking out the resected specimen. Until April 2006, we chose the U-shaped skin incision around the umbilicus, followed by longitudinal incision of the fascia. However, large specimens were often difficult to remore. To solve this issue, we changed the procedure to dividing the umbilical depression at the mid line, and have already performed 48 cases. This procedure enables us to remove the large specimens easily and safely. Since this procedure produces cosmetically better results, we recommend this procedure in totally laparoscopic gastrectomy.
