要旨　近年，低侵襲の重要性がうたわれ，様々な施設で腹腔鏡下胃切除手術の症例数増加が進んでいる状況にある．2004年版の胃癌治療ガイドラインでは，早期胃癌に対する腹腔鏡下手術はStage IBまでを臨床研究として認められた．Kitanoらによる専門施設を中心としたmulti center studyで，早期胃癌の腹腔鏡下胃切除手術の短期長期成績とその有効性が2007年に報告された．JCOGの臨床試験（0703）の第2相試験が始まっているが，その結果によっては，早期胃癌に対する腹腔鏡下幽門側胃切除手術は近い将来，臨床研究段階から日常診療の標準治療になる可能性がある．今後は，適切なRCTを進め，臨床結果に基づいたガイドラインが作成され，普遍的手術となりうる教育システムの構築が必要である．

　Laparoscopic gastrectomy has been performed worldwide, but especially in the Japan. In the Japaneses Guidelines, laparoscopic gastrectomy has been indicated for early gastric cancer（T1N0, T1N1）. Kitano's multi center study indicated that Laparoscopic gastrectomy for early gastric cancer yields good short and long-term oncologic outcomes. To establish Laparoscopy gastrectomy as a standard treatment for early gastric cancer, multicenter RCTs of laparoscopic gastrectomy versus open gastrectomy, are needed for the production of guidelines based on clinical research and education methods.
