急性腹症に対する腹腔鏡下手術の現況

Laparoscopic surgery for acute abdominal emergency today

　要旨：腹部救急疾患の領域でも腹腔鏡下手術が普及してきた．最近の急性腹症に対する腹腔鏡下手術の普及状況およびその適応と意義について述べ，「腹腔鏡下手術手技はあくまでも，合理的な手術を行うための作戦に使えばよい」という考え方を示した．さらにこの領域でもまだ議論の多い「イレウス」に対する腹腔鏡下手術の適応と限界などについて述べた．イレウスに対して手術を実施する場合は，腸管の拡張が軽快していてworking spaceを安全に確保できる単純性癒着性イレウス症例では，まず腹腔鏡下手術を適応としてよい．いずれの疾患においても，腹腔鏡観察による所見，および手術の進行状況によって，小開腹併用あるいは大開腹に移行するなど，術式を適宜合理的に変更していくことが重要である．

　Laparoscopic surgery has prevailed in the field of acute abdomen. The present status and its indications of laparoscopic surgery in the management of acute abdomen have been described in this paper. We believe that laparoscopy should be used as a tool to establish reasonable strategies for diagnosis and treatment of acute abdomen. Indication and limitation of laparoscopic surgery in the management of intestinal obstruction are discussed. Decompression of the intestine and colon is mandatory to obtain enough working space in indicating laparoscopy in patients with intestinal obstruction. In any pathological status, surgeons must not hesitate to convert to small or major laparotomy if necessary, depending on the findings of laparoscopic observation and surgical process.
