消化管穿孔に対する腹腔鏡下手術

Laparoscopic Sugery for Perforation of the Gastrointestinal Tract

　従来，消化管穿孔は開腹術の適応であったが，腹腔鏡下手術が普及した現在では，診断をつけるだけにせよ，はじめから手術の適応と考えるにせよ，まず腹腔鏡下に観察して正確な診断を下し，その所見から，腹腔鏡下に治療を行うか開腹術に移行するかを決定することができるようになった．また，開腹するにしても，腹腔鏡所見から，どの部位にどの程度の大きさの開腹が必要かを的確に判断できるようになった．しかし，腹腔鏡下手術に習熟した外科医でも，腹腔鏡下での操作には限界があることを認識して，開腹移行への閾値を低く保つ必要がある．

Perforation of the gastrointestinal tract is an indication for exploration of the abdominal cavity, and it can be done by laparoscopy today, avoiding large laparotomy incision. Laparoscopic findings give useful information in deciding the indication of laparoscopic surgery, conversion to conventional laparotomy, and its site. Surgeons should keep a low threshold to conversion to laparotomy because laparoscopic surgery has limitations in itself.
