組織拡張器（tissue expander）挿入術への内視鏡下手術の応用

　組織拡張法とは，皮下にシリコン製バッグ（エキスパンダー）を挿入し，徐々にバッグを膨らますことで皮膚を伸展させ，生じた余剰皮膚を用いて再建を行う手術手技であり，おもに母斑や瘢痕切除後の欠損修復などに用いられている．エキスパンダー挿入のための皮下ポケット作製では，皮下剥離や止血操作を小切開から行わなければならないことも多い．そのため，盲目的操作での不十分な止血，剥離がエキスパンダーの拡張過程で血腫，露出といった合併症の引き金になる場合も少なくなかった．そこで，内視鏡下手術手技を本法に導入することにより，安全かつ確実な皮下ポケット作製，エキスパンダー挿入が可能となり，合併症回避にも有用となっている．

Tissue expansion surgery has become a well-established technique, mostly in plastic and reconstructive surgery. A lack of direct visualization during placement or positioning of the expander could be related to the complications (e.g., extrusion). Endoscopy provides many advantages for tissue expansion surgery by reducing some of the complications. It allows the introduction of expanders through mini-incisions placed from a remote site to obtain magnified endosocpic visualization during dissection of the subcutaneous pocket, resulting in minimal trauma to adjacent structures during the insertion of the expander.
