筋弁移植術への内視鏡下手術の応用

Muscle Transfer using Endoscopic Surgery

　筋弁移植術への内視鏡下手術の利用は，筋弁の採取時と採取した筋弁を固定するときに用いられる．特に筋弁の採取では，ドナーの傷を最小限にするという意味では形成外科領域の目的とするところであり有用である．内視鏡下筋弁採取は，おもに広背筋と腹直筋に対して行われている．いずれも腹腔鏡下手術と異なり，術野となる腔（opticalcavity）をつくることが重要であり，そのため内視鏡を固定できるリトラクターを利用する．直視下手術に比べて採取時間が約1.5倍かかることが問題点であるが，術後疼痛が少なく術後回復が早いことや，術後瘢痕が目立たないことなど，その利点を考えるとその適応はさらに拡大されるものと思われる．

Muscle transfer is an important technique for reconstructive surgery. Advances in endoscopy are causing a revolution in surgery that has important implications for reconstructive surgeons. Endoscopic surgery is useful for the harvest of muscle flaps. Furthermore, it is useful for fixation of harvested muscle to recipient beds. We often harvest latissimus dorsi muscle and rectus abdominal muscle using endoscopic techniques for reconstruction. Subcutaneous surgery is different from laparoscopic surgery. The main constraint in subcuta- neous endoscopic surgery is difficulty in creating and maintaining the optical space.
